Payment Policy

• Insurance and Payment: The Client acknowledges that IV MD does not receive payment or reimbursement from managed care plans, Medicare, Medicaid, or any third-party payor, including health insurance, for its services. The Client will be solely responsible for payment, billed directly at the time of service, irrespective of potential reimbursement from any third-party payer. IV MD accepts cash, all major credit cards, care credit, FSA & HSA forms of payment. for all unsuccessful payments in an attempt to collect a debt for services / products rendered, a $35 non-sufficient funds fee shall apply to all unsuccessful transactions per occurrence.
