IV MD agrees to provide the member with a membership program at a cost per month, recurring as a monthly charge. Membership includes IV Rejuvenation, and may include other services such as IV Enhancers, Purified Oxygen and Wellness Boosters per month at a discounted rate depending on membership plan chosen.

When opting for the monthly payment, the member agrees to pay a monthly fee, which will be charged to the credit card on file each month. Any declined charges will result in a $35 non-sufficient funds fee for each occurrence.

The provider reserves the right to amend the membership and its associated fees at any time. The member agrees to provide the provider with up to date contact information so that the provider can communicate with the member.

The member agrees to keep all appointments and to inform the provider at least 24-hours in advance of any cancellations or a $25 cancellation fee will apply.

HOW DO I CANCEL MY MEMBERSHIP?

To cancel your membership, you must submit a written request 30 days prior to your next billing cycle and meet the 3-month minimum requirement. If cancellation is not received within 30 days, you will be billed for the next cycle and no refund will be given.

Memberships including guest are limited to member and one guest of the members choosing for the duration of one month. Member must be present at the time of service. NO EXCEPTIONS or SUBSTITUTIONS. NAD+, and IV Exosome Therapy not included in membership plans. Rules are subject to change without notice. Must present valid ID & membership card at time of purchase and service. Purchases must be made prior to initiating services. Memberships are on auto-renewal and will be charged monthly. You may cancel at any time. For personal and medical safety. IV MD has the right to refuse service to anyone due to certain underlying health conditions that would pose a risk to themselves and or non-adherence of rules or request of IV MD.
